How much does a DOT physical cost?+

Most DOT physicals run $75 to $150. Cash price is usually lower than billing through insurance, and most clinics post a flat rate. Some occupational health clinics charge more if you have complex conditions requiring additional documentation.

How long does a DOT physical take?+

A standard DOT physical takes 30 to 45 minutes once you're in the exam room. Add 10 to 20 minutes for paperwork and waiting. Walk-in clinics during off-peak hours are usually fastest.

What do I need to bring to a DOT physical?+

Bring a valid photo ID, a list of all medications with dosages, glasses or contacts if you wear them, hearing aids if you use them, medical records for any existing conditions (especially sleep apnea CPAP compliance, diabetes, or heart conditions), and payment.

How often do CDL drivers need a DOT physical?+

Standard DOT medical certificates are valid for up to 24 months. Drivers with conditions like high blood pressure, diabetes, or sleep apnea may receive 1-year, 6-month, or 3-month certificates depending on how well-controlled their condition is.

What medical conditions can fail a DOT physical?+

Uncontrolled high blood pressure (over 180/110), uncontrolled diabetes, untreated sleep apnea, recent heart attack or stroke, certain seizure disorders, and significant vision or hearing loss without exemptions can result in disqualification. Many conditions are manageable with proper documentation.

Can I get a DOT physical the same day?+

Yes. Most walk-in clinics on this site offer same-day DOT physicals. With NRII electronic submission (effective June 2025), your medical card is filed directly to FMCSA the same day, so you can get back on the road faster.

What changed with DOT physicals in June 2025?+

As of June 23, 2025, certified medical examiners are required to electronically submit your DOT physical results to FMCSA's National Registry within one calendar day (NRII — National Registry Information Improvement). Drivers no longer need to bring a paper copy of their medical certificate to the DMV — it's transmitted automatically.
